The valve stem gets mounted to the rim with the nut - item #5. The tpms is held in place with the hollow bolt, item #4. The valve stem is independent of the tpms, as in the tpms can be removed without effecting the valve stem seal.

Edit: I'll take a picture in an hour or so when I head out to the garage

The long edge (as viewed from the mounting hole) is on the trailing edge of wheel rotation.
