Inserts do exist, but you will have to get them from a specialty shop. I don't have any in mind right now.

What I have done in the past, is weld a blank over the larger hole then drill and tap it to the size I need. Not sure if this will work in your application or not.

Next thing, is to look at a local machine shop for some direction/help.

If I was broke down on the Alcan I might slap a shim made of folded up flatstock (tin can) on one side of the hole and screw the 1/4 inch bolt into the rest of the hole. Of course you'd have to have the same threads per inch as the larger opening. Hope you're not that desperate but like they say in the bomb squad - improvise.
